Request Access

• Cognos Access Request Form
  Return the completed form to data.warehouse@ocfs.ny.gov.
  You must CC: your Director or Security Coordinator on the email.

For District/State staff only: If the new user should have access to CPS data, please note that on the spreadsheet next to his/her name and ask your Security Coordinator to give them "Access All" in CONNECTIONS.)
