How to Receive Adoption Subsidy Payments (En Español)
The New York State Office of Children and Family Services and Local Departments of Social Services issue adoption benefits electronically via direct deposit or a nationally branded debit card.
By early May 2016, your current JPMorgan Chase Visa debit card will be replaced with a KeyBank MasterCard (shown below):
- The Office of Children and Family Services will send you a notice with details of this change by the beginning of February 2016 and again in March 2016. KeyBank will mail your KeyBank MasterCard Debit Card to you by early May 2016.
- If you have moved and have not provided your Local Department of Social Services with your new mailing address, please contact them. They need to update your mailing address so that your KeyBank MasterCard is mailed to your correct address.
- Any balance on your JPMorgan Chase Visa debit card will not transfer to your KeyBank MasterCard. Use your JPMorgan Chase Visa debit card until you have exhausted all funds on that card. There is no deadline to spend unused funds on your JPMorgan Chase Visa debit card; however, no additional funds will be credited to your JPMorgan Chase Visa debit card after April 20, 2016. Your adoption subsidy funds will be on your KeyBank MasterCard starting with the May 2016 payment.
- If you have any automatic payments set up to draw off your JPMorgan Chase Visa debit card you will need to cancel those payments as funds will not post to your JPMorgan Chase Visa debit card after April 20, 2016.

Option #2: DEBIT CARD:
Payments to your JPMorgan Chase Visa debit card will end as of April 20, 2016
Any balance on your JPMorgan Chase Visa debit card will not transfer to your KeyBank MasterCard. Please continue to use your JPMorgan Chase Visa debit card until you have exhausted all funds on that card.

You can check your card balance at any time for free by calling the number on the back of your card (1-866-742-7506) or visiting the Chase account website: www.myaccount.chase.com
The Chase Debit Card is free, but fees and surcharges may apply to certain transactions and services. Make sure you read the information that comes with your card and keep this information in a safe place.
Any automatic payments set up on your current JPMorgan Chase Visa debit card will need to be ended as funds will not post to your JPMorgan Chase Visa debit card after April 20, 2016.
Payments to your KeyBank MasterCard will begin as of April 22, 2016
